Appointments
We see all patients on an appointment basis and ask that you call in advance so that we may reserve time for you. The office phone number is 317-462--7696. We try to see all patients on time, and request that you extend the same courtesy to us. At least 2 days advance notice is appreciated, if appointment changes are necessary. In instances where appointments are cancelled or failed with 24 hours notice or less a minimum fee of $45.00 may be charged to your account.

Emergency Care
Emergency situations arise, and we will do everything in our power to respond to your concerns. In some instances emergency patients are "worked" into our schedule. Your patience is greatly appreciated. Your comfort is our main concern.

Recare Visits
We believe in the benefits of regular preventive care and encourage all our patients to return for periodic hygiene recare visits. We will send out a written reminder when it is time for your recare visit. When you receive this notice, please contact our office promptly to reserve or confirm your next office visit. At the time of your recare visit, please inform our office team if you have experienced any changes in your health, personal, insurance or employment since your last visit.

Insurance
Please bring your insurance identification card and signed claim form to your first visit. Our office will contact your Primary Insurance carrier to verify coverage and estimate your dental benefits. Dental insurance is intended to cover some, but not all of your dental care. Anv estimated deductible and Co-payment is due in full as services are rendered. We accept most Primary Insurance plans and will file claims for you. Our office will wait up to 60 days for Primary Insurance to respond to claim submission. If payment is not received in full from your carrier within 60 days from date of service, a statement will be forwarded and any remaining balance is due in full from the patient/responsible party. We do not accept assignment of benefits from Secondary Insurance carriers. We will file a Secondary insurance claim for you and ask your carrier to reimburse you directly. We are not a contract dental practice, though some plans will provide benefits if you go to a non-PPO dentist. Please feel free to contact our office with any questions.

Dental Care
During dental visits in which additional dental needs are diagnosed, our office will provide a written treatment proposal for the patient. The treatment proposal lists the dental services needed, the cost of each service, deductible and co-payment estimation due at each visit, and the method of payment the patient/responsible party has chosen. As patient's dental needs change, a new treatment proposal will be provided with updated information.

Payment
Payment in full is expected as services are rendered for all non-insured patients. Patients with verified Primary Insurance coverage are required to pay their estimated deductible and co-payment as services are rendered. After Primary Insurance responds or at a maximum of 60 days from date of service, all balances are due in full by the patient/responsible party. Regardless of pending insurance claims, interest of 1.5% per month will be added to account balances aging to 60 days or greater. If primary insurance can not be verified, payment is due in full at the time services are rendered, an insurance claim will be filed so the patient may be reimbursed directly by their carrier.

We offer several payment options for our patients. A 5% immediate payment courtesy will be extended for payment in full by cash or check at the time services are rendered for any dental services totaling $250.00 or greater. We accept Visa and Mastercard. We offer extended payment plans through an outside resource. If you are interested in extended payment arrangements, please inquire with our financial coordinator.

Office Philosophy

Our team enjoys treating patients of all age groups including those who are apprehensive or fearful of dentistry. We use the latest sterilization techniques and offer nitrous oxide for added comfort. Special Needs
When it is determined that dental care required by the patient is beyond the scope of services of our office, we will refer you to the appropriate specialist. The specialists are selected based on their ability to complete procedures in a proficient caring manner. Payment of fees to these doctors is according to their office procedures.

About Dr. John Neeb
Dr. Neeb was born in Greenfield and graduated from New Palestine High School (1981). He received an Associates degree in chemistry and a Bachelors in microbiology at Ball State University. After working in food and water testing for four years, he entered Indiana University School of Dentistry and then into a General Practice Residency at the V.A. Hospital in Indianapolis. He is a member of the Academy of General Dentistry and received the prestigious Fellowship Award in 2002.
He met his wife Laura while at Ball State University and married in 1985. They have five beautiful daughters: Jessica, Leah, Stephanie, Monica and Bethany.
